  4. All information provided in this application is correct and complete, and I authorize the Bank to confirm or verify this information and my credit record in any way chosen by the Bank.
  5. I understand the salary scheme of the Bank’s sales staff (included basic salary and incentive) is based on staff's performance in financial and non-financial factors and not only the sales target achieved.
